Frequently Asked Question

To find your forms that you need to sign for onboarding, they are found on Payworks. Once you log onto Payworks go to documents on the side of your screen and click on Company Documents.

You will then be taken to a page that has:

• Your Membership Form: Which is a form that confirms whether you want to deduct the membership fee from your paycheck or for you to pay it yourself
• The Federal Tax Form: allows you to claim your federal tax-credits
• The Provincial Tax Form (for Ontario): allows you to claim your provincial tax-credits
• The Payroll Advance Request Form: allows you to request a payroll advance ahead of the pay-day
• The Oath of Confidentiality: confirming that you will not share any confidential information related to the Co-op
• The Video/Photograph Release Form: consenting for images/videos of you to be used for promotional materials for the Co-op
• The Direct Deposit Authorization Form: confirming that you allow the Co-op to directly deposit your earnings into your bank account
• The Accommodation Request Form: requesting that you receive accommodation during your time at the Co-op (which can include disabilities, etc.)

* The forms that are bolded are mandatory for you to complete, the rest are optional/filled as needed

To Download each document. Click on the document that you want to download. A button will appear called Download which you will click to download the document for you to edit it.
